Medication and Substance History

Hunter Serotonin Toxicity Criteria

How Doctors Treat Serotonin Syndrome

The appropriate level of treatment depends on the severity of symptoms.

Supportive Care

Treatment may include:

  • Oxygen and airway management when necessary
  • Intravenous fluids to maintain hydration and support kidney function

  • Cooling measures for significant hyperthermia

Standard fever-reducing medications such as acetaminophen are generally not effective for serotonin-syndrome hyperthermia because the dangerous rise in temperature is driven largely by excessive muscle activity rather than the same mechanism that produces fever during an infection.

Benzodiazepines

Cyproheptadine is a serotonin antagonist that has been used in the treatment of serotonin syndrome and is discussed as a treatment option.

It may be considered when symptoms persist despite initial supportive treatment. Because cyproheptadine is available orally, administration and dosing depend on the patient's condition and clinical circumstances.

Importantly, the evidence supporting cyproheptadine is more limited than the evidence supporting immediate discontinuation of serotonergic agents and supportive care. Dosing approaches are not completely standardized, and decisions about its use should be made by medical professionals.

A retrospective study reported clinical improvement following cyproheptadine treatment, but evidence still comes largely from case reports, observational studies, and smaller patient series rather than large randomized clinical trials.

Benzodiazepines do not directly reduce serotonin levels but can be important for controlling agitation, excessive muscle activity, and other symptoms while the body clears the serotonergic substances involved.

Other medications used to treat different hyperthermic syndromes are not interchangeable with serotonin syndrome treatment, which is one reason accurate diagnosis matters.

Who Is at Highest Risk of Serotonin Syndrome?
