Program Name: Scottsdale Providence Review Request
Description: Scottsdale Providence Recovery Center sends SMS text messages to clients who have opted in, containing a link to leave a Google review of their experience.
Message Frequency: 1-2 messages per consent period.
Message and Data Rates: Message and data rates may apply. Contact your wireless carrier for details.
Opt-In: By providing your mobile phone number and signing the SMS consent section of the intake form at Scottsdale Providence Recovery Center, you agree to receive up to 2 SMS messages containing a review link.
Opt-Out: Reply STOP to any message to opt out. You will receive a confirmation message and no further messages will be sent.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.

When you opt in to our SMS review request program, we collect your mobile phone number solely for the purpose of sending you a link to leave a Google review. Your phone number will not be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes. Your mobile information is stored securely in our internal system and is accessible only to authorized Scottsdale Providence staff.
